  • Referral slip from your medical doctor, if you have one
  • Your medical insurance card
  • A copy of your medications list, including dosages
  • If you are getting therapy as a result of a workers’ compensation claim you will also need to bring your claim information, including claim number

Comfortable clothing is always recommended, such as loose pants or shorts, a T-shirt or tank top and comfortable shoes or sneakers.
Most Private Insurance will cover Direct Access to Physical Therapy Services in North Carolina without a physician referral. Peak Performance is committed to ensuring our patients care is coordinated with their overall medical needs and conditions. When treating patients without a physician referral we will request permission to contact the patient’s physician in order to coordinate treatment. Please contact one of our Physical Therapist at (866) 953-1278 for further information and specific recommendations.
As a service to our patients, we gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card (to ensure that we have the necessary information.)
Most major insurance coverage is accepted, such as: Blue Cross & Blue Shield, Tricare, Medicare, and Workers Compensation.  Your insurance information will be requested before your first appointment. That is done to verify your benefits and coverage.
Payments are accepted via cash, check, debit and credit card. Payment is expected at the time of treatment (normally during check-in). If you have insurance, there may be a deductible or co-pay required.
Policy states that only patients are allowed in the treatment rooms. For safety reasons, please make arrangements for your children. This also allows us to focus on your treatment.
